Welcome! To get started, please select your desired provider in our scheduling system to book your New Patient appointment. Please note that acute and speciality visits are for established patients only.

Dr. Pinazza now requires a referral from your provider. You may email it to psr@wildhorsehealth.com with the subject line "New Patient Referral" or fax it to 480-470-1748 after scheduling your new patient appointment. If you do not have a referral, please schedule with another provider.

We are a cash-pay practice, and our providers are out-of-network with most insurance carriers. However, we are happy to provide you with a superbill if you wish to submit your own claim to insurance after your visit. Please note that insurance reimbursement for our services is not guaranteed and varies depending on your specific carrier and plan.
Our medical services are typically covered by flexible spending (FSA) and health savings accounts (HSA). Please inform your provider and provide the necessary paperwork if your plan requires documentation for coverage.Department of Pain Management & Rehabilitation

Pain follow-up: A routine and treatment-focused visit for any concern that a Wild Horse Health provider has previously addressed in the past (This is most common.)

Comprehensive follow-up: An extended visit for any concern that has NOT been previously addressed by our providers. This allows us extra time to diagnose your condition and come up with a new treatment plan.

Comprehensive follow-up: Should be scheduled for any routine general medicine follow-up.

Short follow-up: Must be scheduled by our office and approved by your provider. This visit is not available to patients scheduling in the pain management department
Please note that patients who have already been seen by a Wild Horse Health provider in one department of our clinic must schedule a new patient appointment when seeking services in another department. This is to allow time for a specialized intake process and evaluation.
Follow-up visit policy: We reserve the right to change your appointment type based on the level of work-up required for your chosen health concern(s). Provider-made changes to your visit cost will be explained to you during your visit and require your consent. Please be aware that declining these changes will lead to adjustments in your care to reflect your preferred appointment type.
We have several follow-up options for established patients. Acute visits should be booked for acute injuries and illnesses only.

New Patients: If you cancel or reschedule your appointment within 24 hours of the scheduled time more than twice, a fee of $100.00 will be added to the cost of your new patient appointment.
Established Patients: If you cancel or reschedule your appointment within 24 hours of the scheduled time more than twice, or if you have an excessive number of total cancellations or reschedules, you will be liable for the total cost of the missed appointment. These charges will be added to the fees for your next visit.
